Skin botox and an ECM skin booster in Korea, explained by the dermatologist who treated her: why textured skin needs both, what ECM means, and VOS prices.

I’m Dr. Hongseok Kim, a board-certified dermatologist with 21 years in practice and the founder of VOS Dermatology in Gangnam. The short answer first: skin that looks textured and stiff, even under makeup, usually needs two things at once. Skin botox smooths the surface by calming oil and fine texture, and an ECM (extracellular matrix) booster supports the dermis underneath so the skin feels more supple. Neither changes your features. What Heli noticed about her own skin
In her post, Heli described it plainly: before treatment, her skin looked very textured and rigid, even with makeup on. That is one of the most common descriptions I hear from international patients, and it is a useful one, because it tells me the problem is not only dryness and not only pores.
I don’t share a patient’s private medical details on this page. What I can share is the reasoning I apply to anyone who sits in front of me with the same complaint, because the logic is the same.
Texture that shows through makeup tells me the surface is uneven: small bumps, visible pores, areas that are oilier than others. Rigidity tells me something about the layer underneath, the support that lets skin move and reflect light softly. A routine that only hydrates the top layer rarely fixes both, which is often why people arrive having already tried many products.
A 1:1 consultation before any plan
Heli wrote that one of the things she liked most was that the doctor sees you personally, and that she had a 1:1 consultation where her skin was studied carefully. That is how VOS works. I look at the skin first, in good light, and ask what bothers you day to day. Then I explain what I would do, and what I would leave alone.
She also mentioned that the staff were kind and that English interpretation was available. For many visitors that matters as much as the treatment itself. You should understand every step before it happens.
Skin botox: smoothing the surface
Skin botox, also called micro botox, uses the same medicine as wrinkle botox in a different way. Instead of a few deeper injections into muscle, I place very small, diluted amounts just under the skin surface across many points.
At that depth it mainly affects the oil glands and the tiny fibres attached to the skin. Patients notice skin that looks smoother and less shiny, with pores that appear tighter, while their expressions stay natural. The effect generally begins two to three days after treatment and reaches its full level at about two weeks. I explain the technique in more detail in my skin botox Korea guide.
What ECM means, and why it fits stiff-looking skin
ECM stands for extracellular matrix. It is the framework in the dermis, the structure that keeps skin firm, elastic and supple. When people say their skin feels rigid or less bouncy, that layer is part of the story.
ECM boosters are regenerative injectables built around that framework. On our official list, the acellular dermal matrix (ADM) boosters are Juveàcell and Re2O, micronized dermal matrix products aimed at dermal regeneration, elasticity and density. Worth knowing: A regenerative booster works with your skin’s own repair, so it builds gradually. If you need a visible change for an event next week, tell me. I would rather plan around your date than promise something the treatment does not do.
Why both, in the same visit
Think of it as two layers. Skin botox handles the surface: oil, pore appearance and fine texture. The ECM booster works underneath, on the support that keeps skin supple. Treating only the surface leaves the stiffness; treating only the support leaves the texture. Heli wrote that both treatments were adapted to her skin, and that is the point. The products are standard; the plan is not.
There is a practical side too. Both are injected in one sitting, so a short trip only needs one recovery window. Small bumps from the injection points are normal and usually settle within a day.
Aftercare is simple. For the first few hours after skin botox, don’t lie down or press on the treated area. On the day itself, skip saunas, alcohol and intense exercise, and avoid rubbing the injection points. Use sunscreen every day for the following week. These small steps help the skin settle so you see the treatment, not the marks.
How I would plan this for you
Every plan starts with an in-person consultation. Here is what I check:
- Surface or support. Oil and enlarged pores point toward skin botox; stiffness and loss of suppleness point toward a regenerative booster. Many people have both. If pores are your main concern, my pore treatment article goes deeper.
- Your schedule. Book early in your trip, not the day before your flight, so any bumps have time to settle.
- Your expectations. If the real issue is sagging, a skin booster is not the answer, and I will say so.
